PERKESO LINDUNG 24 JAM 2026: Should You Opt In or Opt Out?

PERKESO LINDUNG 24 JAM 2026: Should You Opt In or Opt Out?

A Comprehensive Protection & Value-for-Money Analysis by Biztrak

Malaysia’s social security landscape reached a major milestone with the launch of PERKESO LINDUNG 24 JAM (also known as the Skim Kemalangan Bukan Bencana Kerja or SKBBK). Enacted under the Employees’ Social Security (Amendment) Act 2026, this scheme bridges a long-standing protection gap.

Previously, SOCSO/PERKESO only covered accidents occurring at work or during work commutes. If an employee suffered an injury during a weekend football match, a household repair accident, or personal holiday travel, PERKESO provided zero coverage. LINDUNG 24 JAM changes that. It extends coverage to non-work-related accidents 24 hours a day, 7 days a week within Malaysia.

However, PERKESO announced a critical policy update: Participation in LINDUNG 24 JAM is voluntary for local Malaysian employees, while remaining mandatory for foreign workers. Malaysian employees face a crucial decision before the 31 August 2026 opt-out deadline: Should you opt in or opt out?

At Biztrak, we believe informed financial choices build workplace resilience. Here is our detailed evaluation of LINDUNG 24 JAM, its pros and cons, how it stacks up against private medical cards, and the final value-for-money verdict.

1. Fast Facts: How LINDUNG 24 JAM Works

Before deciding, understand the mechanics of the scheme:

• Who pays? The contribution is 100% employee-borne via monthly payroll deduction. Employers facilitate the deduction and submission.
• Wage Ceiling: Contributions are calculated on monthly salaries capped at RM6,000.
• Phased Rates:

• Phase 1 (June 2026 – May 2028): 0.75% of monthly salary (Max RM45/month).
• Phase 2 (Years 3–5): 1.00% of monthly salary (Max RM60/month).
• Phase 3 (Year 6 onwards): 1.25% of monthly salary (Max RM75/month).

• No Age Limit: Coverage continues as long as you remain employed, even after age 60.
• No Medical Underwriting: No pre-existing condition checks. No medical health screenings required.

2. Key Eligibility & Employment Conditions

Crucially, eligibility under LINDUNG 24 JAM is strictly tied to your active employment status:

• Active Employment Required: Protection under LINDUNG 24 JAM follows the period of an employment. You are covered only while actively employed under a contract of service.
• No Coverage for the Unemployed: If you resign, face retrenchment, or are currently unemployed, you are not covered during the period of unemployment. Coverage resumes when you secure new formal employment.
• Multiple Employers Rule: If an employee has more than one employer, they do not pay double. The employee can choose which employer will handle the LINDUNG 24 JAM payroll deduction via PERKESO's Portal LINDUNG Faedah. (If no selection is made, PERKESO will assign one based on set criteria).
• Self-Employed Personnel Excluded: Self-employed individuals, freelancers, and gig workers are not eligible for LINDUNG 24 JAM. Instead, self-employed individuals can register for PERKESO's SKSPS (LINDUNG Kendiri) scheme, which is specifically designed for self-employed social protection.

3. Opting In vs. Opting Out: Thorough Evaluation

Option A: Opting In (Staying Covered)

Advantages of Opting In

• Unbeatable Affordability: At a maximum of RM45 per month during Phase 1, you receive extensive financial protection. No private personal accident policy offers lifetime disability pensions at this price point.
• Income Replacement Benefits: If an off-duty accident leaves you temporarily unable to work, PERKESO provides Temporary Disablement Benefits (daily cash payouts) during your sick leave.
• Long-Term Disability & Dependant Pensions: In the event of permanent disablement or fatal non-work accidents, PERKESO pays monthly pensions to you or your eligible dependants (spouse, children, parents).
• Full Implants & Rehabilitation Support: Private medical cards often limit coverage for medical implants or physical rehabilitation. PERKESO covers high-cost orthopedic implants, prosthetics, and complete physiotherapy under its Return-to-Work programme without a strict monetary cap.
• No Universal Exclusions for Pre-Existing Conditions: Unlike commercial insurers, PERKESO accepts all active employees regardless of age or medical history.

Disadvantages of Opting In

• Direct Salary Deduction: It reduces your monthly take-home pay by up to 0.75%–1.25%.
• No Illness or Disease Coverage: The scheme covers accidents only. It does not cover medical illnesses such as heart attacks, cancer, stroke, or dengue fever.
• Restricted to Panel & Government Facilities: Free medical treatment is primarily delivered through government hospitals and PERKESO panel clinics. It does not cover private hospital executive suites.

Option B: Opting Out (Exiting the Scheme)

Advantages of Opting Out

• Higher Net Take-Home Pay: You retain up to RM45–RM75 per month in your salary packet.
• No Duplication for Fully Protected Individuals: If you already possess top-tier private disability insurance and personal accident coverage with high income-replacement riders, you save money.

Disadvantages of Opting Out

• Irreversible Loss of Safety Net: If you opt out and suffer a severe non-work accident (e.g., a weekend highway crash or slipping at home), you forfeit all PERKESO cash benefits, pensions, and rehabilitation support.
• Private Cards Don't Pay Salary: Private medical cards pay the hospital bill; they do not replace lost monthly salary if you cannot work for six months.
• "Once Out" Risk: Under current guidelines, opting out is a significant decision. However, opting out is reversible — PERKESO allows you to apply to rejoin the scheme by submitting an OPT-OUT to OPT-IN request.

4. LINDUNG 24 JAM vs. Private Medical Cards & Commercial Insurance

A common misconception among employees is: "I already have a private medical card from my agent or employer, so I don't need LINDUNG 24 JAM."

This compares two completely different financial tools.

• Medical Cards are designed to pay hospital admission and surgical bills at private medical centers.
• LINDUNG 24 JAM is a social safety net designed for income replacement, rehabilitation, and long-term disability pensions.

Here is a side-by-side comparison:

5. Protection & Value for Money Verdict

Which option provides better protection and value for money?

The Verdict: Complementary, Not Mutually Exclusive

• For Lower to Middle-Income Earners (B40 / M40): Opting In is an absolute necessity. For less than RM45/month, no commercial insurance product can match LINDUNG 24 JAM’s lifetime disability pensions, dependants' benefits, and free physical rehabilitation.
• For High Earners with Existing Private Insurance: Opting In remains high value. Private medical cards pay the private hospital, but LINDUNG 24 JAM ensures your family continues receiving monthly cash income if an off-duty accident prevents you from earning.

Value Recommendation: Stay Opted In.

Treat LINDUNG 24 JAM as your baseline social security shield for income replacement while employed, and use your private medical card for comfort in private hospital rooms.
